Rates:
Fees are available upon request.
Insurance:
Dr. Bellace’s services may be covered in full or in part by your health insurance or employee benefit plan. She can provide you with invoices which you can submit to your insurance company for out-of-network reimbursement.
Payment:
Personal checks and cash are accepted for payment at the time of each session.
Cancellation Policy:
If an appointment is missed or cancelled with less than 24 hours notice, you may be billed for the session.
